Procedures for paper forms

To receive any assistance offered by our University, it is necessary to register at the Accessibility Center.

STEP 1

Please complete the registration form (pdf). After printing and signing, the form should be personally delivered to the Center along with a copy of medical records confirming your health condition (original for verification). In exceptional cases, it is possible to send documents by registered mail to the following address:

The registration form is completed only once (unless your health condition changes).

STEP 2

Please complete the application for support (pdf). In this form, you will find all types of support offered by our University. Once printed and signed, the application should be delivered to the Center.

WHAT NEXT?

1. Within 7 days from receiving the application signed by the decision-making bodies, the Accessibility Center shall inform the student via email or telephone about the support granted (or not).

2. A student receives the decision on the granted support from the Accessibility Center.

3. A student independently decides whether to inform a teacher of their rights.

In exceptional cases related to the student's health and at the student's request – the Center will inform a teacher of the student’s rights.

4. A student has the right to appeal the decision. The appeal can be made in writing (via post or e-mail) within 14 days from the date of receipt of the negative decision

In cases concerning the refusal to grant entry to the internal parking lot, adaptation of materials, provision of teaching materials or adaptation of the conditions for credits and examinations, appeals should be made to the Vice-Rector for Development and Cooperation with External Environment.

In other matters, in particular those listed in Order No. 174/21 of December 15, 2021, appeals should be made to the Rector of the University of Economics in Katowice.
