Procedure for organizing internships

All correspondence between students and academic staff shall be sent from accounts in the @uekat domain. The Dean responsible for internships is Prof. Joanna Palonka, Ph.D. – Vice Dean of the School of Undergraduate and Graduate Studies.

By signing an agreement, a student declares that they possess valid Personal Accident Insurance (NNW).

Documents should be delivered by email 2 weeks before the planned internship.

  • all documents must be sent in one email,
  • screenshots of correspondence must include uncovered/visible email addresses of a student/Supervisor/representative of an internship host.

1. Mandatory internship (outside of the University)

Step 1. A student, in consultation with an internship host, completes the Agreement on  Mandatory Student Internship.

Step 2. A student obtains the signature and seal of a representative of an internship host. If it is not possible for the host to sign and provide a seal for the document, please, send an email confirming the admission to the internship and the name of the authorized person at the host institution (the email address must be sent from the internship host’s domain to verify the credibility of the confirmation).

Step 3. A student sends the completed Agreement to their Supervisor electronically, attaching a confirmation from an internship host that they agree to accept the student for the internship. After reviewing the submitted documentation, the Supervisor agrees to the mandatory internship through their signature on Agreement (4) or via email.

Step 4. A student emails the Agreement along with the consent of an internship host and their Supervisor to the address provided in the email containing the required forms attached, writing in the email subject line: AGREEMENT. The student receives information on the approval of a mandatory internship from the Dean.

Step 5. A student completes their internship.

Step 6. After completing their mandatory internship, a student provides the Mandatory Internship Completion Form signed by an internship host and their Supervisor. If it is not possible for the internship host to sign and provide a seal for the document, please, send an email confirmation of the internship and the student's performance, along with the name of a person authorized by the internship host (the email address must be sent from the internship host’s domain to verify the credibility of the confirmation).

Step 7. A student sends the completed Mandatory Internship Completion Form via email to their Supervisor, attaching an internship host’s confirmation. After reviewing the submitted documentation, the Supervisor credits the practice on the completed Mandatory Internship Completion Form or via email. The student sends the Mandatory Internship Completion Form together with the approval of the internship host and the Supervisor to the address received in the email with the forms attached.

Step 8. A student receives information that they were granted credit for their internship.

2. Mandatory internship (in the University’s organizational unit)

Step 1. A student, in consultation with the University’s organizational unit providing an internship, completes the Declaration of an Organizational Unit of the University on the Admission of a Student to a Mandatory Internship.

Step 2. A student obtains the signature and seal of an internship host.

Step 3. A student sends the completed Declaration to their Supervisor electronically. After reviewing the submitted documentation, the Supervisor agrees to the mandatory internship through their signature on Declaration (4) or via email.

Step 4. A student emails the Declaration along with the consent of their Supervisor (on the Declaration (4) or via an email) to the address provided in the email containing the required forms attached. The student receives information on the approval of a mandatory internship from the Dean.

Step 5. A student completes their internship.

Step 6. After completing their mandatory internship, a student provides the Mandatory Internship Completion Form signed by an internship host and their Supervisor.

Step 7. A student sends the completed Mandatory Internship Completion Form via email to their Supervisor. After reviewing the submitted documentation, the Supervisor credits the practice on the completed Mandatory Internship Completion Form or via email. The student sends the Mandatory Internship Completion Form together with the approval of the internship host and the Supervisor to the address received in the email with the forms attached.

Step 8. A student receives information that they were granted credit for their internship.

3. Recognizing other activities as a mandatory internship

Step 1. The Student completes the Completion Form Recognizing Other Activities as a Mandatory Internship.

Step 2. A student obtains the signature and seal of an internship host on the Completion Form Recognizing Other Activities as a Mandatory Internship. If it is not possible for the host to sign and provide a seal for the document, please, send an email confirmation of the activity and the student's performance listed on the Completion Form, along with the name of a person authorized by the host (the email address must be sent from the host’s domain to verify the credibility of the confirmation).

Step 3. A student sends via email the Completion Form Recognizing Other Activities as a Mandatory Internship to their Supervisor, attaching the host’s confirmation. After reviewing the submitted documentation, the Supervisor approves of the learning outcomes achieved during other activities on the Completion Form Recognizing Other Activities as a Mandatory Internship or via email.

Step 4. A student sends the Completion Form Recognizing Other Activities as a Mandatory Internship together with the approval of a host and their Supervisor to the address received in the email with the forms attached.

Step 5. After obtaining the Dean’s approval, a student receives information that they were granted credit for their internship.
