• Consent of the Dean for a traineeship mobility within the Erasmus+ program -  The document requires the signature of Professor Anna Sączewska-Piotrowska
• Confirmation of the traineeship acceptance by the company
• Traineeship Agreement ("Before the Traineeship" part shall be completed. The Traineeship Agreement must be signed by the student/recent graduate, the host company, and Professor Anna Sączewska-Piotrowska in the case of student traineeships, or by Patrycja Macioł, the Erasmus+ Institutional Coordinator - in the case of graduate traineeships.)
• Travel declaration

During the mobility:

• Interim traineeship report

After the mobility:

• Travel tickets (to and from) -  only in the case of the previous application for green travel (to and from)
• Traineeship certificate - TA
• EUSurvey (the survey link will be sent by email on the day the internship ends)
